Adderall vs. Ritalin: Revealing the Differences

Adderall and Ritalin are the two most prescribed stimulant drugs for ADHD [1]. Both have a similar action in increasing the amounts of dopamine and norepinephrine within the brain, which helps one focus and block impulses. They are not identical medications, though, and they also have different precise contents, rates of action, lengths of action, and recommended usage [1] [2].

This article describes the differences between Ritalin and Adderall in terms of ingredients, duration, effect, and medicinal use.

What are Ritalin and Adderall?

They are both medication stimulants for ADHD [1]. Ritalin is a brand name for the drug methylphenidate, and Adderall is a brand name for a combination of amphetamine salts (dextroamphetamine and amphetamine) [2]. Both drugs increase norepinephrine and dopamine levels in the brain, enhancing attention, focus, and impulse control. Essentially, they do roughly the same thing overall – accelerate brain function – but with somewhat different chemical active ingredients and remarkably similar mechanisms of action [1] [2]. Neither of the medications cures AD/HD, but both function as treatments when taken under a doctor’s supervision.

How do Ritalin and Adderall function?

Both enhance norepinephrine and dopamine, but Adderall also causes further release of neurotransmitters [1] [2]. Ritalin (methylphenidate) most effectively by blocking the reuptake of norepinephrine and dopamine back into the nerve cells so that more of the “attention” chemicals remain in the brain. Adderall (mixed amphetamine salts) blocks reuptake of the same neurotransmitters and forces nerve cells to release even greater quantities of them. In practice, both drugs’ signals pass through gaps (synapses) in the brain, which otherwise would be blocked by ADHD, and improve concentration and reduce impulsiveness [1] [2]. The impact of Ritalin in everyday life can differ from one individual to another. Some individuals may find Ritalin more effective in their case, while others may be more responsive to Adderall [1].

How quickly does Ritalin and Adderall work?

Both Ritalin and Adderall begin to affect the body 30-60 minutes after consuming a dose. Generally, you will start to feel the effects of either medication in less than one hour after taking it. Ritalin IR will begin to take effect within 30 to 45 minutes, and Adderall IR within the same timeframe. Ritalin peaks in the blood a little sooner, typically at 2 hours post-dose, while Adderall peaks a little later, at 3 hours [3]. The long-acting (extended-release) version of either drug takes a little longer to work. 

How long does each drug last?

Adderall will last longer than Ritalin, as the active compounds remain in the body for a more extended period.

The immediate-release (fast-release) form of Ritalin typically calms the symptoms for 3-4 hours, after which its effects begin to wane. The duration of a dose of Adderall IR is usually about 4-6 hours [3]. This translates to needing to take more Ritalin throughout a typical day to sustain it. In contrast, Adderall may sustain larger blocks of time with fewer doses.

Both of these medications also have longer-acting (ER) versions that are designed to last for many more hours. Ritalin LA or other ER preparations will sustain 8-12 hours, and Adderall XR sustains 12-16 hours. In real life, take one morning dose of long-acting in the daytime, or divide a short-acting dose between the morning and later afternoon.

What are Ritalin and Adderall dosage forms?

Ritalin is more commonly available in various forms than Adderall. Ritalin (methylphenidate) comes in multiple preparations, including immediate-release (IR) tablets, chewable tablets, liquid solution, extended-release (ER) capsules and tablets, and a transdermal skin patch (Daytrana). Examples include Ritalin SR, an extended-release tablet; Ritalin LA, an extended-release capsule; and Daytrana, a transdermal skin patch that releases methylphenidate into the system.

Adderall is more memorable in this regard: it comes in both immediate-release tablets and extended-release capsules (Adderall XR). (Also, Mydayis, a long-acting amphetamine preparation, but standard Adderall packaging is IR tablets and XR capsules.)

  • Ritalin formulations: IR tablets, chewable tablets, oral solution, ER tablets/capsules, and a transdermal patch.

  • Adderall formulations: IR tablets and XR capsules.

Choosing a form is based on the duration of the effect required and personal preference. Ritalin is available in various forms to facilitate accurate dosing, including a chewable pill and a patch that provides sustained delivery. In contrast, there are fewer forms of Adderall.

Who typically takes Ritalin versus Adderall?

Age and patient considerations determine the drug of choice. Ritalin is approved for use in ADHD in children 6 and above and adults [4]. Adderall is approved for ADHD in children 3 and above and adults [3] [6]. In practice, pediatricians will place younger children (below 6 years) on Ritalin (or its extended-release forms) if patients need to be medicated, with amphetamine formulations like Adderall being used more in older children and adults.

Studies show that methylphenidate-containing drugs have been given and attempted at an earlier age in children; therefore, pediatric practitioners may prefer to prescribe Ritalin as a better option for extremely young patients.

Both Adderall and Ritalin are first-line drugs in adults and adolescents, though more Adderall is prescribed to adults. This is at least partially because studies have found amphetamine derivatives (like Adderall) to be perhaps especially beneficial in adult ADHD. Lastly, a physician makes a decision based on a patient’s history, age, symptoms, and response history to drug and side effects.

What are the common side effects of Adderall and Ritalin?

They have many of the same side effects in common due to their being central nervous system stimulants. Either medication has the following side effects in common:

  • Loss of appetite and weight loss (most often recognizable, particularly in children) [5].

  • Sleep disruption (insomnia) in case a dose is administered too late in the day [5].

  • Nausea or stomach ache (particularly in the beginning of treatment) [5].

  • Headache or dizziness (mild) [5].

  • Increased heart rate or elevated blood pressure (mild, but potential).

 

Both of these side effects are usually manageable (e.g., taking the dose a little earlier to prevent insomnia) but need to be monitored by a physician. Both carry a warning of potential growth suppression in children and should be used with caution [6].

There are a few differences. Adderall will be more likely than Ritalin to produce mood change or irritability. Irritability was more commonly seen with Adderall in children in an extensive review. Rare but damaging side effects (with either drug) may be massive increases in heart rate, new or worsening psychiatric side effects (e.g., agitation or anxiety), or even psychosis (hallucination), particularly if they are used recreationally [5]. An extensive study found that the rare risk of psychosis was likely greater with amphetamines such as Adderall than with methylphenidate. Because Adderall is longer-lasting, its effects may also be longer-lasting than Ritalin’s side effects.

What drug interactions should I know about?

Ritalin and Adderall also share similar interactions, except for a few. In general, steer clear of other stimulant medications or any medication that significantly raises heart rate and blood pressure. Some examples include decongestants found in certain cold products, antidepressants, or weight-control medications. Use these stimulants too, not with MAO inhibitors (older class of antidepressant medicines), because that is not safe and strictly contraindicated.

Ritalin and Adderall also interfere with anything that raises serotonin (because of possible serotonin syndrome) and with anything that increases dopamine or norepinephrine [3] [4]. Both alcohol and excessive caffeine should be avoided or used to an absolute minimum with either medication because both potentiate jitteriness and insomnia.

There are some Adderall-specific interactions: potassium citrate (a drug/supplement) delays the elimination of Adderall, and hence its action lasts longer than desired. At the same time, vitamin C (tart foods) reduces amphetamine plasma levels, which might counteract the effect of Adderall. None of the specific interactions occur with Ritalin.

Always inform your physician of all prescription and over-the-counter drugs you take before you start taking Ritalin or Adderall. Your physician will monitor the interactions and adjust the doses as necessary. Due to such interactions, physicians generally do not prescribe Adderall and Ritalin concurrently.

Are Ritalin and Adderall addictive?

Neither drug is typically habit-forming when appropriately used for ADHD, but both do have abuse potential [1].

Both Adderall and Ritalin are Schedule II controlled medications in medical practice because they pose an abuse risk. But studies show that in the hands of individuals who actually do have an ADHD diagnosis, they aren’t that addictive in comparison to illegal drugs [1]. That is, a kid or an adult who takes Ritalin or Adderall isn’t going to be chemically dependent just on that pattern of use [1]. The issue comes when such drugs are not used by the people with prescriptions or are abused.

Teenagers or others may get high off Ritalin or Adderall to cram the entire night or get through their reading; this is when addiction and severe side effects set in [1]. Psychological dependence (depending on the impact more and more) can be a result of either drug when it is abused and can have highly detrimental side effects such as high blood pressure, heart issues, or psychosis [1]. Both Ritalin and Adderall rely on a potential for dependence when they are abused. Doctors are therefore careful in screening patients before prescribing and monitoring their use. Neither drug is more “addictive” than the other when they are being used appropriately; abuse potential is equatable, and more a matter of what route the drug is being taken versus what medicine it is [1] [2].

Can I take Ritalin and Adderall simultaneously?

No – neither should ever be taken simultaneously.

Physicians never co-prescribe Ritalin and Adderall for ADHD since they would only be doubling the dose of the stimulant and thereby doubling the risk of its side effects. If one drug isn’t controlling symptoms sufficiently, the standard strategy is to increase the dose of that drug or change medicines, not to add another stimulant. In real life, if a patient does not respond to Ritalin, the physician will switch to Adderall (and vice versa) [2].

Is one better in children or adults?

It is age-specific. Ritalin (methylphenidate) is primarily used in younger children, while Adderall is more commonly used in teenagers and adults. To illustrate a specific case, physicians will typically initiate Ritalin or other methylphenidate in preschool children (ages 4-6) initially before they proceed to amphetamines. In school-age children, they are both first-line drugs. Still, the vast majority of adults are placed on Adderall, depending on how they react to amphetamines.

Can I use Ritalin and Adderall together?

No. Using more than one at the same time isn’t recommended, as it has a high likelihood of significantly increasing side effects. If medication is ineffective, physicians will prescribe more medication or switch to another therapy rather than continuing to take it at the same time.
